The proposal aims to empower elders in Hong Kong by integrating health technology to enhance health monitoring and chronic disease management for individuals with borderline hypertension. The project will utilize wearable devices, mobile applications, remote monitoring systems, and data analytics tools to enable early detection of hypertension-related health issues, provide real time data for chronic disease management, and empower elders to make informed decisions about their health. The project will involve nursing school students who will provide assistance, education, and care coordination to the elders. The goals of the project are to improve health monitoring, facilitate chronic disease management, engage nursing students, and improve elder health literacy.
